There's nothing complicated about the following rule proposal:

"As a member of the rules committee I will do everything in my power to get the rule changed for 2019 and beyond so that when a die leaves the playing surface (perimeter of the board) it is dead and the roll doesn't count! This will remove all sorts of possible ambiguities." Neilkaz

If the die bounces out and comes back in you just roll again. I can honestly only think of ONE occasion where I have seen this, which is the die hitting the glass as mentioned. If a player knocks it back in somehow you just roll again; no need for either player or the TD to concern themselves with whether there was any intent to do so.

Simples!
