My guess is it is actually very easy to get into such a situation.

If you gain $150 by losing, you don't need the total amounts you can possibly win to be less than $150, only your current equity in the tournament.

For example let's say you start 0-3 in a Swiss. Given that you would need to win 8+ matches in a row to cash, your equity in the tournament is incredibly low. If others in the pool are winning (or even if they aren't), you are probably better off throwing your next match.
