The first early-game exception that comes to mind is something like 54S-41. Of course you hit 6/5* with the 1, but I see a lot of players continuing with 6/5*/1*. Along similar lines, I will often see 52S-52 played as 8/3*/1*. Perhaps somewhat paradoxically, players from the pre-bot era are less likely to make this kind of error.
