There is a further potential ambiguity here.

Yes, there have been enough additions/changes to the rules that they could use a complete rewriting.

Suppose I have

7/6, 6/5*, 6/5, 5/4.

Following your first rule, I have created only one asterisk. Now when I start merging, the ambiguity is whether I should merge 7/6 with 6/5* or with 6/5.

Maybe it would be better to ignore the asterisks at first and add them at the end. So

7/6, 6/5, 6/5, 5/4 -> 7/4, 6/5

Then add the asterisk for the hit. In "7/4", here is an implied intermediate move to the 5-point, so it gets the asterisk.

7/4, 6/5 → 7/5*/4, 6/5

(I think if I did a rewrite, I could explain this in a cleaner way.)

I don't really understand your example

6/5, 6/5*/3 → 6/5*, 6/3

Once the asterisk comes off "6/5*/3" you get "6/5/3" and then a serial merger is possible since the intermediate point no longer hits. (This suggests you may have to loop back and reapply some previous rules, though maybe there is a way to word the rules so that everything gets done in a single pass.)
