This is what Sam said, basically. This was the case in Pat Gibson's LA OPEN where the last defeated player in the main, (Dorn Bishop in that case), fed into the final 5. The final 5 INCLUDES the undefeated finalist.

Losers in that round of 4 players to determine the once defeated finalist were places 4 and 5. The winners played and the loser took 3rd place, while the winner of the 4 player bracket made the ultimate finals.

In LA, one match was played there to determine the Champion...

In Michigan it is true double elimination, and the undefeated player has to be beaten twice.
