Director's first priority should be to find out what actually happened. He can spend time interviewing the players. He might want to ask spectators as well. Those answers, combined with his own good judgment, should form the basis of his ruling. Only as a last resort should Director use as a crutch the idea that one player "caused" the problem, and maybe not even then. We all slip up sometimes.
Getting back to Joe Russell's story where the unbiased third party was forced to finally admit he was only 99% sure the dice were 5-5. If Dubious Character continued to press the point, I would making my ruling thus: "Okay, I've written a number between 1 and 100 on this piece of paper. If you (Dubious Character) can guess the number I've written, I will rule the roll was 5-4. If you don't guess correctly, my ruling is that the roll was 5-5."
