Taper Mike: "When LM is in force, a player should be warned when he misplays the first two or three or four times. After that, if the pattern continues, the director should be allowed to impose a penalty."

When LM are not in force the opponent is allowed to impose a penalty on those occasions that (he feels) the careless illegal move was to the detriment of the player making it.

But that is a regressive perspective, of course...
