How would a world-wide rating system work?

To pick a country at random, suppose everyone in Ghana just plays other Ghanians. Maybe the Ghanians are fantastic players or maybe they are terrible players.

If they only play amongst themselves, how could their internal win/loss records tie into a world system in any meaningful way?
