1. You wouldn’t. On catching an illegal play, you have a choice:
i) Immediately pause the clock, and get a TD to enforce the penalty.
ii) Ask the ask the opponent to make a legal play, without involving any director. The downside for the offended player here is that if you decide later in the match to consult the director to resolve a legal moves dispute, he can only enforce the illegal move penalty from the point where he is called to enforce.
2. This is a detail. The bottom line is that a prerequisite to establishing illegal move penalties is to get every match clock mandatory played with one set of dice. The focus should first be to pass these prerequisite standards…then move on to the specific details as you mention. The pushback I have heard about clock mandatory is a joke…you have 5-year old chess players with ratings under 600 using clocks with no issue…pretty sure open-level BG players can manage.
3. The penalties are 100% objective, and should be applied uniformly by the TD. The main responsibility of the director is to understand the mechanics of the approved clocks well enough to add/subtract time according to the established penalties.
