Clocked BG -- missing the fact that it's your turn

Player A makes a checker play, but leaves it in the final position for a long time before hitting the clock.
Player B doesn't realize that Player A has hit the clock, and just sits there looking at the board.
Player A doesn't say anything, because in his mind, it's Player B's turn, and assumes Player B is thinking about the cube. (Assume it's reasonable to think a long time about the cube.) Five minutes goes by before Player B realizes it's his turn.
Scenario 1: Player B had ten minutes originally, now he's down to five. Tough shit?
Scenario 2: Player B had just over five minutes originally, now he's down to 30 seconds. Tough shit?
Scenario 3: Player B times out. Tough shit?
At what point (if any) should Player A say or do something?
