I have seen dice land one on top of another.

By the way, the picture on the home page of our NYC Meetup group shows a die that appears to be stopped. But who knows what the shutter speed of the camera was.

As for incidents that stick out in ones mind from long ago that haven't happened since (unrelated to cocked dice)... When I first began playing lots of backgammon in college, I played in two games within the same year in which a player got gammoned (once giving, once receiving) without ever having a checker on the bar the entire game. It has been 25 years or so and I've never played or even seen this happen since then.
