I would definitely break (moving both checkers off) the 18pt. Many years ago I played a similar prop where breaking the 18pt with a 42 was slightly best (a brilliant play that Malcolm played OTB, with experts dissenting) in spite of the fact that White's resulting non-breaking position was less awkward than here. It was for money with Blue holding a 2 cube, but I find it hard to imagine that 18/16 18/14 would be wrong at DMP.
The cube is in the center. If it's money Jacoby, White should have doubled before rolling 42. So, let's say it's a money-type score in a match, where White could reasonably have perceived it was too good to double. In that case, I would stack up the 3pt because it gives Blue fewer numbers to get back in the game: only 11 61 instead of 42 41 21 22 11.
So, in answer to your question: "Yes," the fact that it's DMP does enter into the decision -- at least for me. At DMP, I break the 18pt, but for money (either Jacoby or in a match), I stack on the 3pt and let the cube do my talking.
