Seeking clarification on dice tube usage...

Besides Mr. Meyer's website, is there any easily accessible documentation of rules/guidelines of dice tube usage in a tournament environment?

Is it within the rights of the tube advocate to force a roll-off to determine whether or not it gets used in the match?

If a player on roll flips the tube but does not place the tube down flatly (whether on purpose, or unintentionally), and the player then flattens it out after the dice reach the bottom with a twist of the wrist, should the roll stand?

If a player repeatedly does not place the tube down flatly as described above, and the opponent must repeatedly call for a ruling, would it be unreasonable of the Director to remove it from play, despite any pre-match roll off or verbal agreement made between the players?
