For me, these are easy:
1) Over, over, and over. If you resign a match, and your opponent accepts, the match is over.
2) No ethical obligation. Let me put it this way, it is ethically preferable to point this out.
I would point it out, but don't think there is an obligation to do so. I think BG is more enjoyable if you give your opponent the chance to correct such a mistake.
3) Same answer as above. For me, this one is easy, because I nearly always rule to conclusion so probably would not be aware there was a mathematical chance still.
What I am not clear about is whether the rules allow you to decline a resignation which you feel is in error, so I expect some people to come in and comment about that.
