1. Good catch. Duh. Well, it was a long time ago and whatever the score was, the doubler (leader) redoubled to a cube level he couldn't use. Was he 2-away? 1-away post Crawford? I'll trust my memory on that, even if I can't apparently recall the exact score.
2. Again, call a director and I will lay heavy odds my illegal action of disallowing A to cube will be upheld every day of the week. That's why the first rule exists
This is such a tired, tired argument. Been going on 20 years now. Rules writers can't write rules that make sense, that are consistent, that are followed, that the TD's themselves even follow -- and why not? Because "they know" "what the rules mean," and there's always Rule 1.1 to fall back on.

Maybe so. All I can say is, if they rule that you, playing "legal moves," can disallow the bad double, then they must also rule that the cube action is illegal. In fact, they must rule that you must disallow the cube action (if you weren't playing "legal moves," you'd have the option). Because surely they aren't going to rule that you have the right to allow your opponent to replay perfectly legal actions. Or are they?
