It's all a matter of power. If you're a sufficiently powerful and famous author, then you can throw your weight around and get your way. Otherwise, contracts are generally written to favor the publisher. The publisher generally has the right to choose the title. The author might argue with the publisher, but can't force matters.

In many cases, the author basically signs away almost all rights to the book, and can't even get the copyright to the book back even if the publisher lets the book go out of print.
