OT - Poker Fail - Rollout

This was handled poorly and anyone who says otherwise makes my top 5 donkey of the week list. They already knew he had pulled the same bullshit earlier in the tournament. What should have happened?
- The first time it happens you're given a warning. Verbal declarations are binding and you will be forced to min raise, that's common knowledge.
- The second time it happens your shit doesn't float. You are sent to the rail for at least 10 minutes as your chips get anted/blinded away.
- Third time, thanks for playing, you may leave now and never come back
That is me being nice and trying to adhere to the rules without punishing the player who is clearly angle shooting from hand #1 too much. What should be written into the rules or otherwise enforced is that if you pull that move of putting the correct amount of chips out to call and announce raise and turn over a 'nut like' hand your raise is void, you win that pot but can sit on the rail the first offense. Any repetition of said offense you're out of the tournament.
One of the things wrong with letting this guy do this is that in many tournaments the floor wouldn't even be called over and the tournament staff wouldn't know about his previous unethical behavior. Every dealer knows the verbal action is binding and could enforce it himself though hopefully upon seeing how the hand unfolds he would also know enough to call the TD over and make sure he saw what went down.
Not that we should put up with these kinds of actions in backgammon either but poker has so many fish losing this one jackass will adversely affect nobody.
