Championship, Advanced, Intermediate: Clock Preference (one player can decide to clock the match)
Novice, Beginners: Clock Option (both players can agree)

That is the most common rules, however individual tournaments may state other clock rules.

At the final rounds of the big tournaments, such as Danish Championships, Nordic Open, etc. Clocks are mandatory.

Side-events, such as jackpots, are typically Clock Preference, except for specific limited beginner jackpots, that are Clock Option.

Doubles are always Clock Preference.

Clock settings used are always 12s+2m/p, except for doubles (15s+3m/p) and speed.

As for Speed, it can vary, and it up to the tournament organizer, but DBgF maintains that in order for the matches to get rated, you must at least have a 10 second delay, and 20 seconds per matchpoint in bank time. The most common setting is the same as Chiva describes, 5 point match with 10 seconds per move and 2 minutes in the bank.

As for the big Team Leagues, you have Clock Preference in ALL divisions (since about 6 years ago, before that, only in the top divisions).

Denmark is a small country, so even though we are many active players, we do get to know eachother. That also works for slow play, ie. you know who you need to put on the clock, and who plays fast. So when you look at a tournament room, sometimes maybe only a third of the matches are clocked, make no mistake about it the majority of players would pull out their clock if they faced one of the known turtles.

It's also my feeling, that our tournament directors are more alert (in the monitoring sense) about slow play, and relatively quickly will point out to players if their match are hanging up the tournament (and put a clock on that match, if the first warning didn't produce any result).

Finally I think I should notice that, IF you want to submit your match to the DBgF Tournament Committee for review (to achive a norm), your match MUST have been clocked. Non-clocked matches are simply not submissible. In general, the bigger the tour, the higher the clock percentage. That also goes for different flights.
