The issue you raise of who is causing the harm is an interesting one, albeit perhaps somewhat semantic.

On this basis I suppose, it could be questioned whether a drug dealer is causing the harm to an addict, or the addict themselves. (Of course, dealing in banned substances is illegal, whereas playing bg for money, in certain circumstances, may not be.)

Alternatively, whether the partner of a severely (bedridden) obese person is the one doing the harm by continuing to supply massive amounts of food, or whether it is the obese person themselves.
