1. The rebuy cut-off is usually a time that's a couple hours after the first round begins, somewhat based on match length.
For example, in this June's Los Angeles Open, the main round started right around 8pm, and rebuys closed at 10:15pm. A good "rule of thumb" for estimating how long a match should take is to give 10 minutes per point of match length. Matches were 11 points, so this gave enough time for almost all matches to finish and the losers time to decide if they wanted to rebuy.
2. Typically, it's paired with single elimination formats, but I've seen it used with double-elimination formats as well. The most common way of utilizing a rebuy format is to do a Round 1 draw that is totally separate from the main bracket. Any winner earns their place in the main bracket. Losers have the option to get a second "Round 1" opponent by rebuying, or just going to consolation.
After the rebuys close, anyone that won their initial match or rebuy match are placed in the (fresh draw) main bracket. Those that did not win a match are placed in the (fresh draw) consolation bracket.
It is possible to rebuy twice in most formats if both of your two losses are fast enough. If there is an odd number of rebuys, the odd person out does not get a rebuy match.
