In the games I have played...
A) Usually, player 2 is partner, and player 3 is captain. If either box or partner leaves the game permanently, losing captain becomes partner. Sometimes, players are given a choice, in order, but this can disadvantage some participants.
B) Box keeps his partner (except as covered by A, sentence 2).
C) Their place in the original order is irrelevant. Box and partner go to the bottom and next-to-bottom of the order, one way or the other (as predetermined).
There are many variations.
To Bob's point, I have played interlocking chouettes with as few as four players and three-game interlocks with as few as six players, no consulting, one cube per game (you can pass a captain's take but can't take his pass). It's a lot more action and (I think) a lot more fun.
