1) Can you take a timeout in the middle of a game? If so, what are the clock "mechanics" to do this?
You can never take a timeout in the middle of a game. Inbetween games you're still alloted your one 5 minute break but per rule 2.2. (ABT Tournament Rules)
2) Related to question 1, can you stop the clock to write down a position or take a picture of a position for future study?
No, this is part of the clock that I like. If you want to take a picture or write down a position you do it on your time. That means nobody can bitch any more about the taking of pictures since you're using up your time to do so.
3) When matches are clocked, are there a fixed number of "breaks"?
Again, Rule 2.2 above. People are pretty lenient if you need another break for a real reason though. In other words, if you're taking breaks to talk on your phone or smoke I doubt anyone is going to let you take more than one, but if you're going to the bathroom or grabbing something to eat/drink, nobody will care.
4) Assuming the standard is 12 seconds per move and 2 minutes per point, is time added back to your total remaining time if you take less than 12 seconds to make a move?
No time is ever added for moving faster than the 12 second buffer.
