1. I'm OK with Rich's answer here.
2. Is there a digital clock that doesn't have a warning feature? I guess some may only warn when time expires. I'd say no.
3 & 4. Um, lol, no.
Analogous situation: a player runs out of time, then argues that he didn't know that running out of time meant losing the match, so tries to claim that he hasn't in fact lost.
Of course I'd rule that the player lost since he ran out of time, but I also honestly believe that this player has a more legitimate claim than the donk in 3 & 4.
Chiva: "We usually set it so it sounds only when total time is over. It's the players responsibility to check on their remaining time and manage their speed."
IMHO it's also the player's responsibility to call a flag. I would rather it not be set at all when I play.
Personally, I would find it distracting for the clock to be beeping and/or flashing at me. I know how much time I have left, I don't need something else (a kibitzer, a clock, a director, an opponent, whatever) to highlight that point. A warning doesn't actually speed up your pace, it just alerts you to the fact that there is a clock and you have X time left...which is what you would know in the first place?!
Anyway, consider how much discussion this has generated over what is an interesting question but also a practical triviality. Turning it a point of contention for clock use in general would IMHO be pretty fuckin' stupid.
