The fact that A may or may not have errored in giving the cube does not change the fact that player B indisputably gave up 7% MWC by passing when he should have taken. Player B's error must be 7%.
As for Player A, it seems clear also.
If B correctly takes, A has given up 5% MWC and should be penalized 5%. If B passes, A has seemingly made a correct choice to double and the size of the error like for any other correct cube decision is therefore zero. By definition, "correct" is the decision that gains the most equity. You don't get negative error for gaining equity - it is simply a non-error.
