"To gain it back you need to:
1. Reach a position from which you will double. This may involve sacrificing additional equity in many intermediate moves in which you refrain from doubling.
2. Your opponent must make a cube error."
In fact #1 is not necessarily required. The way to gain equity back is to have your opponent's position improve to the point where they miss their own market and double you out, or perhaps they simply forget to ever double (not that any world class players would ever make that mistake in a major tournament).
I consider these scenarios sufficiently likely that if I am an underdog and my opponent has seen potential market losing sequences and has not doubled, I will not double either if I see a market losing sequence in my own favor. I don't consider this giving away equity; quite the contrary. In such a situation, my opponent has already giving away equity, and if the position favors them, they are likely giving away more equity each turn they fail to double than I am. So maximization of equity is achieved by continuing to make small mistakes to permit the opponent the chance to make bigger ones.
