Elderly players and time clocks

"One thing I'm relatively certain of is that any accommodation should be granted no later than the beginning of the match. The idea that the director would be summoned to grant extra time once a player is already in time trouble strikes me as dreadfully arbitrary."
I can certainly see some person not wanting to make waves and thinking they can handle the situation and discovering otherwise. I think it is too rigid to say that the director's good judgment cannot apply here when it can apply anywhere else. That said, I would think that directors should generally limit a mid-match adjustment to a change in the per-move delay time rather than adding more reserve time back to the clock. If more delay time would have been granted at the beginning of the match, I see no problem at all making the same exception later. The penalty for not speaking up earlier is the loss of whatever reserve time has already been expended.
