What are the worst playcalls in the history of football?

What is your assumption regarding the odds of making the FG?

If we assume, for the sake of argument, that it was physically impossible for the kicker to make the FG due to injury, what is play B? I'm guessing on 4th-and-medium it was correct to go for it.
