I agree with Bob that this is not really that rare an occurrence. Based on how often I've seen it arise in a real game, I'd estimate that something like this comes up about once every few hundred games (that's games, not matches). This is one of my favorite types of positions because it is so badly misunderstood by otherwise reasonably strong players. You can gain quite a lot of equity with the cube with a little study.
