There is one other element that I believe should be expressly stated and understood by all relative to disputes. In the event there are gray areas, or it is not clear exactly what happened or what ruling should be made, the tournament director should tend to make the ruling that is most fair to the player that did NOT initiate the action which created the problem or question in the first case.

In order words, when in doubt, penalize the one who dropped the dice cup. Penalize the one who hit the table; the one who set the clock wrong; the one who reached for the doubling cube; the one who made a potentially misleading comment; etc.

So if there is doubt as to whether the dice were rolled properly or not, the fairest ruling would be to allow the opponent to decide if the roll should stand or go over. The other player might not like his opponent having such a choice, but it was his fault for not rolling properly and causing the incident.
