My ruling would be to make it known to each player about the clock policy and that this is their "warning", let the match proceed to the finish.

I think it is as much a lesson to the staff, and I realize my opponent in USBGF International Online event this year is graciously having the event as his home, and I don't mean disrespect by posting.

I think more defined procedures for the staff/TD to catch this earlier are more appropriate. A lesson learned.
