Your D and O family designations are correct:

    D = 13/9(2)
    d = 13/11(2) 13/9
    D = 13/11(4)
    d = 13/11 13/7

    O = 13/11(3) 8/6
    o = 13/11 13/9 8/6
    O = 13/5

Nactation is primarily designed for the early game, generally where both sides still have at least one back checker. In this position, White has escaped both back checkers, and therefore Blue is unusually inclined to make blotty plays on his side of the board.

Even so, in practice one needn't resort to italics. The O play can be nactated $ (Slot), and the d play can be nactated % (alt slot). [You're stuck with D for 13/11(4), but it is a horrible midpoint-breaking play nobody would make.]

In your table template, I suggest you combine the "Made Points" and "Distance from 6pt to Made Point" columns into one column entitled "Distances of Owned Points from 6pt" (thereby handling the most-points convention and the top layer of the 6pt convention together). With points, the key aspect is not checker destination (as it is with blots/spares); rather it is a matter of the points you OWN when the play is completed. [Your entries in that column will be any points not commonly shared by all plays listed. For example, in the D table: 3 7, 5 7, 5, 7; the higher-ranked plays have two points and the lower ranked plays have one point.]

Consider entering "--" (or the like) in the Distance from 6pt to Nearest Blot/Spare Destination" column (or eliminating the column altogether) whenever the tie has already been broken by a higher aspect. The information is irrelevant. [Thus, the three entries of +1, 5 and 3 in the second row of the D table would be reduced to a single entry of 5 7.]
