1. If B chose to roll too quickly then yes make him pay the price of allowing A the chance to see his next roll. Only way people will roll properly is if there is a penalty.
2."Player B should return his dice to the table with the appropriate numbers and await Player A's completion of play." No fofeit, but I think Player A should get to decide whether B is stuck with that roll or not. It's a way to make people wait if they know they will be stuck with a crap roll. I reiterate: Only way people will roll properly is if there is a penalty.
3. a)Can't see anything else to do about the numbers. b)"But, again, I assume he has forfeited right to cube on his next roll?" I would say yes.
4)I think that's a perfect way to render a correct decision and personally would be very happy with that. However, when playing live I am very careful with my rolls being rolled at the right time (AFTER my opponent has picked up his dice). The only time I have ever rolled too fast is with a friend in a friendly game and at the end when it's a roll off and after I have seen his roll. Oh, and when partying and it doesn't matter to anyone and sometimes the dice are already blurry. :)
