At what point should a player just be kicked out of the tournament?

I don't think that's an answer you can generalize. It depends what the extent of the rudeness is. Some rudeness can be overlooked, other such actions a warning given (then perhaps a ban if it happens again), and to the other extreme some stuff I feel you should be banned on the spot, all of it is up to the TDs discretion of course.
