Most frequently broken US rule?

Does hitting the clock with the dice cup or the borne off checkers qualify as a hand?
And if the player uses the wrong hand, is it still his turn? And can you then count pips for 12 seconds, hit the clock and say "you hit the clock with the wrong hand, please correct that to properly complete your turn"?
And does running with an opening 64 without touching the checker to either the 20 or 18 points on the way to the 14 point constitute ambiguous?
And has a player that rolls 26 from the bar, and plays the bar/18 24/23 broken this rule? I've seen that a number of times.
