One reason for this particular error may have been insufficient optimism! It's easy to see that you aren't totally out of the woods yet, since shots are possible. But balance the caution in your mind in that regard by being aware before your roll of what your best shots are, too! This isn't done in a bean-county way, or in a way that costs any real time or mental CPU cycles. Just a general awareness is all that is needed. You need to be at least vaguely cognizant that there are some good rolls available, not just bad (big dubs esp. 33, bad 2s) and temporary badness-avoidance (double-bigs that aren't doubles).
