1. I would rule according to the letter, and let black decide whether white should reroll, furthermore I would state loudly that "I would like to thank Black for requesting a ruling on this point, as it illustrates an important principle: You should not roll early! For in doing so you may put yourself at the mercy of your opponent in positions like this, and you have no guarantee that your opponent will be a man of honor and let your roll stand, for he might instead act like a despicable bastard and require a reroll even if it will earn him a bad reputation for years to come."
2. In most cases I would just let the roll stand. However I tend to be a bit picky on fast rolling (in order to not end up in these situations), so if White was a notorious fast roller, she may have gotten numerous requests from me during the match to stop doing this. In that case I would at least be very very tempted to go with the despicable bastard option, in order to teach the lesson properly.

4. As above, my opinion of Black depends a bit on what has passed. As in the story, if the match has progressed without Black complaining about fast rolling, I would hold it against him if he requires the reroll. Of course, just requesting the ruling to make a point, and then allowing the roll to stand is perfectly cool.
BTW, BIBA used to employ the rule that all premature actions stand, which would make this and a number of other potentially awkward decisions no-brainers. Is there any good reasons why that rule hasn't caught on?
Also, is this from an actual event, in that case, what happened?
