Sounds like you want a description of the draw mechanics for each round. Here is what we do in Madison.

We keep a card for each player. On each card, we record their opponents for each round and the result. As each round finishes, we mark the cards and pile them according to record. The four-loss player cards are discarded, because these players are out of the tournament. When it is time to do the draw:

  1. If there are an odd number of players left, we take the pile of worst-records, remove anyone who has already been given a bye and shuffle them. We draw one player to get the bye.
  2. Then we start with the pile of best-records. We shuffle and begin making pairings.
  3. When we finish one pile, we move on to the next-best.
  4. If there are an odd number of cards in a pile, we just leave the last card and draw an opponent from the next pile.
  5. As we put two cards next to each other, we check to see if the players have played before. If yes, we draw a new opponent for each of them. In late rounds, this can get complicated. Sometimes we have to back up and redraw a match or two. We first try to force a draw that keeps like-record players together, but if necessary, we will make more pairings using cards from the next pile to avoid repeat pairings.

Notice that all 3-loss players stay active in the Swiss. Then when we switch to a playoff format, the 3-loss players go into a consolation bracket and the 2-or-fewer-loss players go into the main playoff bracket. We also feed losers from the main bracket into the consolation bracket "progressively" so that players with the same number of wins are always playing each other.

As you can see from this discussion, it isn't perfect but it works.
