"My approach (and I'm not a director) is that before director creates a harsher than standard penalty, the player should be warned of the infraction previously by the director. I think it is irrelevant that the opponent has said something previously just among the players, and it's irrelevant what the kibitzers saw or think about this. If an opp fast rolls, either agree between you to adjust literally to the rule (above, in the US) or call the director. Otherwise the opponent of the fast roller is effectively condoning it."
I fail to see why the director is the one that must inform the player about the rules. If the opponent says you are violating the rule, the player either needs to accept the new knowledge about the rule and conform, or seek out the director to clarify the rule. The rules are there in print for all to see. Ignorance is no excuse. It is reasonable for the player to give his opponent a warning. It is also reasonable for the player to impose the rule on the first offense regardless of his opponent's ignorance. A player warned by his opponent cannot then claim ignorance when the director arrives. He should have no expectation of a warning from the director. It is a very different thing if the opponent had remained silent about the behavior all match long and finally when it matters, he claims that it is improper. Then it is condoning the behavior. But if each and every time the opponent says "what you are doing is illegal - I am going to be nice and allow you to reroll this time, but be warned I may require your roll to stand next time", I don't think the player should have a leg to stand on when he fast rolls the next time and the director gets called.
