Regarding ratio of golfers worldwide to golfers in the US, the answer to that may be on the web, too. Here's something related:

As I'm sure you know, I was trying to get a ballpark number so I could have put in quite a few values there and still reached (at least qualitatively) the conclusion I did.

First, golf is a game biased towards the wealthy. Secondly it's a game with origins in Scotland that propagated almost exclusively through the British Empire (maybe there's a better word/term for that -- "Commonwealth"?). Now golf has gained popularity in countries that aren't spinoffs of GB, such as Japan and Korea, but even there I'm thinking it's not played by the masses.
