How rare is this?

I suspect such an (undetected) double hit happens every several games, or something like once every other day over all of MLB. Of course winding up on base (and credited with a hit) would surely be much rarer -- maybe a few times per season? And I would expect those to be infield hits, so this one (outfield single) could be ("ballpark" :) a once-per-season occurrence.
Those are just my WAG's. Anyone else?
In order to make an accurate estimate I think one would be helped quite a bit to know how often a bat is broken. (I don't know the answer to that, but I'd guess 2 or 3 per game.)
