Tipping etiquette...

One point I haven't seen made explicitly yet regarding tipping in general: in the US, some employees receive a wage (salary or hourly compensation) from the business owner that assumes the worker will receive tips. Waiters at restaurants are a good example of that. I don't know if Europe is different (probably). As such, sometimes giving a tip is mandatory (unless you really want to go out of your way to express dislike for services rendered...).
Tip jars annoy me, too. So do cashiers asking for a donation to their (or the company's) favorite charity. Another HUGE pet peave is waiters NOT pointing out that a mandatory gratuity (typically 18%) was added to a restaurant bill (because the number of people served was large) so that you will mistakenly leave a double tip.
Finally (and back to Justin's original question), I wonder how people feel about the tipping that occurs (or doesn't occur) at poker tournaments. Is there a parallel with tipping the staff at BG tournaments?
