We once saw an error that appeared to be > 20%. It was the last play of the game and a 54-yard field goal could have been attempted (when trailing by less than 3), but the team tried a pass play. (I'm pulling this out of my memory so the details could be off a bit.)

That one is tougher to analyze because 1) it's hard to get a really good handle on the probability of making a 54-yard field goal -- few have been attempted; and 2) it wasn't great weather at the time -- wet and cold if I remember correctly.

It also likely was windy, given that it was Neil's favorite team who screwed up -- the Chicago Bears playing in the Windy City. :) (But Neil, don't blame Lovey because this was during the rein of a previous head coach.)

We have found a handful in the neighborhood of 15%. So this is right in there amongst the worst.
