I agree with Walter Trice about prime vs prime positions posing some of the most difficult problems in backgammon. Checker play in a backgame or in a position where both sides have many checkers back can also be very challenging.
More specifically, I have found certain anchor-breaking situations very tough to handle. This is especially true when the checker running off the anchor is exposed to unduplicated shots. The alternative to breaking anchor might be making a big positional concession such as crunching the inner board.
In a more general way, I agree with Bob's comment about choosing the right game plan. In the early stages of the game where neither side has made much structure, almost any plan seems to be plausible to me. Only after the position develops further I can feel more sure (and committed) about following a particular plan.
So far I haven't mentioned any cube problems because I think there are very difficult cube action situations in almost areas of backgammon and I cannot rank them in order of difficulty.
