Is every instance of holding or someone being offsides a lack of rules knowledge also?

It was a difficult play that wasn't executed well enough. How easy do you think it is to know exactly how far the ball has gone on the field when

i) you are running near full speed

ii) the ball is moving also and possibly bouncing erratically

iii) you can't really take the time to get a view exactly from above to eliminate any parallax effect

iv) you have a helmet on with a facemask

v) there is imminent contact with several large men and their locations and movements also warrant observation.
