I believe that your comments refer to improving bg skill-set which contributes to online and live PR while I refer to errors/blunders that I have the tools to deal with but manage to miss-play in live matches. If I make those "unforced errors" (a term I borrowed from the tennis world but with a different context that I use for bg self study purposes) I investigate them in roughly the same way you described and try to find the reason that caused it. I reach a conclusion like you did but one that doesn't improve my bg game skills, yet makes me play better in live matches. The following position, taken from a match in Novi, is a good example to illustrate my point:

20/14, 6/3* is clearly best here in order to maximize coverage of the outfield. Nonetheless, it leaves 2 extra blots around the board. Given the score, and holding the 2 cube your instincts tell you to be slightly conservative by playing 21/15 and not maximizing the coverage of the outfield. In the past I would have miss-played it even tough I knew the correct play. This time, I played it correctly but used some clock time to fight the conservative instinct.

I still see good players miss-play such positions. You can say that the difference in equity is insignificant. This is a weak argument because with small changes it would give a lot of equity. Some say it is "adjusting" to their opponent. I find this funnier because it is no more than an excuse.
