I don't know what should have happened in your situation. I know that if the score is 2-1 to 9, and it turns out the match really should have been to 11, the common-sense solution is for the director to make them play to 11. I assume Phil is not suggesting otherwise. We could have a philosophical debate on exactly when it becomes "too late" to make the players play to the correct score.
The more challenging case is where the players have already played past the proper score when the error is discovered. Rewinding the match and declaring that the winner is whoever reached the correct score first may sound good in theory, but in practice it's a wholly arbitrary solution. Sometimes it will completely reverse the result reached over the board, sometimes it will have no effect whatsoever. Both players are responsible for the error, yet one of them often receives a huge windfall while the other pays a staggering price. Phil's solution satisfies those of us who believe there ought to be some penalty, while allowing the match to be decided over the board rather than by a director's ruling.
The reality is that this situation occurs far too infrequently for there to be any deterrent effect, but it still seems clear that there has to be some kind of penalty.
