a) The late arrival penalty helps keep the tournament on schedule, so I think it is clear that this should not be optional, for the players to agree on.

b) I personally think it is more logical to use the clock to regulate late arrivals, rather than penalty points. At the designated start time, get someone from tournament staff to help roll the roll, and start the clock.

Also, the tournament should probably require players to be present at 5 min before designated start time. Then a player possibly without an opponent can ask tournament staff for assistance prior to the starting time. This will also allow for some time to try to contact the missing player.
