Illegal moves are unintentional most of the time, but when you find yourself against someone who makes a lot of illegal moves, and most of them puts him in a more favorable position, then you are facing a cheater. And unfortunately, this is not rare in money games and in lower divisions of play.
I have found that in the Open Division, generally the players have such high respect for the game and their opponents, and are concerned about their reputation, that virtually all illegal plays are accidental.
When you do find a player who tends to make illegal plays in his favor, then your antennae should go up and you need to be extra careful not only about how the player moves, but how he rolls, how he keeps score, and what equipment he is using. People are rarely dishonest only in one area.
