I think you cannot force someone who is sick to play. Obviously, that creates a risk that someone unwilling to play, for whatever reason, will claim to be 'sick'.

And that again is a general problem with rules and exceptions - often the exception to a rule deal with a situation that most will recognize deserves an exception, but can be exploited by the non-scrupulous.
